Clinical Threshold Policies

(Click “Show More” to display full list)
Clinical Threshold Policies are evidenced based pathways which are used by clinical specialists and general practitioners (GP’s) which provide clear criteria for eligibility for referrals and clinical treatments.

These clinical policies are reviewed and updated by Norfolk & Waveney ICB taking into account the most recent clinical evidence such as NICE, Evidence Based Interventions programme, and also seeking the views and expert opinions of Public Health Consultants and clinicians working in primary and secondary care settings.

In instances where a clinician believes that a patient would benefit from a treatment, however, does not meet the clinical threshold criteria an Individual Funding Request (IFR) application can be submitted by the treating clinician outlining the patient’s exceptionality.

GP referral forms can be found on Ardens or in the Healthcare Professionals section
