Shared Care Agreements 

THRESHOLD

• If the patient meets the policy criteria refer direct to NHS provider via e-referral.
• If the patient does not meet the criteria then referrer can apply for funding in exceptional circumstances.

NOT ROUTINELY FUNDED 

• Requires approval via the Individual Funding Panel for exceptional circumstances.
